Insurance reimbursement

Submitting a claim to insurance is surprisingly easy! I will provide you with a superbill after services (bariatrics/SCS clearance/autism evaluation) are rendered. All that is generally needed after that is to sign into your account on your insurance company's website, click "submit claim online," fill out requested information, and upload the superbill.

Blue Cross Blue Shield NM

1. Submit a claim online

Submitting a claim online is the most efficient method. After logging in to your secure member website, follow these steps:

  1. Click "Claims Center," then "Submit claims"

  2. Complete your claim online

  3. Copy, scan and upload your supporting documents, including itemized bills, original receipts

  4. Click "submit claim" to complete the process

  5. After you submit your completed claim form, you will receive a notification by e-mail to confirm that it has been submitted successfully.

Aetna NM

1. Submit a claim online

Submitting a claim online is the most efficient method. After logging in to your secure member website, follow these steps:

  1. Click "Claims Center," then "Submit claims"

  2. Complete your claim online

  3. Copy, scan and upload your supporting documents, including itemized bills, original receipts

  4. Click "submit claim" to complete the process

  5. After you submit your completed claim form, you will receive a notification by e-mail to confirm that it has been submitted successfully.

2. Submit a claim by fax, email or traditional mail

You can also submit a claim by traditional mail, email or secure fax. Mailing addresses and fax numbers are included on your form for your convenience. Don't forget to make copies of everything you mail for your own records.

After logging in to your secure member website, follow these steps:

  1. Click "Forms," from the navigation menu

  2. Download the form you need

  3. Complete the PDF or Word document online OR download, print and complete the form by hand

  4. Attach all necessary supporting documents, including itemized bills, original receipts, certificates and X-rays

United NM

1. Submit your claim online

Sign in to your health plan account to find your submission form

Sign in to your health plan account and go to the “Claims & Accounts” tab, then select the “Submit a Claim” tab. There, you’ll be able to select the Medical Claims Submission form to download and print.

2. Submit your claim by mail

After you print and complete the Medical Claims Submission form, mail it with the claim details and receipts to the address on your health plan ID card.

Helpful hints

Here are some tips and tricks on how to make the claims process easy and efficient.

  • Be sure to complete all of the applicable fields on the form. Ask your provider for the Provider Information or have them fill it out for you.

  • Keep a copy of the form, claim details and receipts for your records.

  • Send the claim as soon as possible, and as close to the date of service as possible.

  • Complete a separate form for each claim.

  • If you have other insurance or Medicare and it is primary to your UnitedHealthcare plan, include that corresponding Explanation of Benefits (EOB) with your claim.
